Supporting documentation

As part of the application process, you will also be required to upload supporting documentation.

Aim to provide documents which are current, relevant, and concise.

The supporting documentation required includes:

Evidence of your disability (i.e. Autism diagnosis) * You need to provide a letter from your doctor that confirms your diagnosis and indicates the level of impact your disability has had/will have on your studies

Evidence of your connection with WISE Employment * You need to provide a letter from a representative of WISE Employment that confirms your registration with their organisation.

Declaration * I declare that the information supplied by me is to the best of my knowledge correct. I understand that giving false or misleading information is an offence under the Criminal Code (Commonwealth). I understand my application will be assessed based on the information I have provided.
I agree to attach current and relevant documents only and I understand my application will not be assessed if I fail to provide documentation by the due date. I acknowledge documents I may have uploaded for previous scholarship applications will not be considered with this application.
I understand Deakin University is collecting my personal information on this form for the primary purpose of determining my eligibility to receive a scholarship. Where applicable, Deakin may also provide this information to external scholarship donors for the purpose of selection, which may include participation in an interview process, and may use de-identified application information to assist with analysis and development of future scholarships. I am not required to provide the information requested, however if the information is not provided, Deakin may not be able to determine my eligibility to receive a scholarship.
