If you have been contacted to provide information relating to pay remediation, please provide details using the form below.
This secure form has been established to facilitate the exchange of all relevant details, help coordinate all claims for remediation and process any outstanding payments. You will be asked to provide bank account and superannuation fund details as well as supporting documentation to verify your identity. If you would prefer to confirm your details over the phone, please call Deakin HELP on 03 5247 9999 (9.00am - 5.00pm, Monday to Friday).
Note that this form is only for former employees who have received communication from the University notifying them of a specific underpayment for remediation and asking them to verify their details. It is not to be used to make new claims.

Frequently asked questions
Why do I need to verify my identity?
If your personal contact details and/or bank/superannuation details have changed since you were last employed at Deakin University we will require support documentation to confirm payments are made to the correct individual.
What if I am unsure if my payment details have changed?
If you are unsure if your payment details have changed, upload your verification documents by selecting that your details have changed.
How will I receive payment?
The payment will be made to your nominated bank account as a lump sum.
Casual Payment Claims
Former casual staff members can submit historical payment claims where they believe they have been underpaid, including where they:
- were requested to undertake additional work but did not submit a timesheet or receive payment
- submitted a timesheet and believe it has been incorrectly processed
- wish to seek a review of a claim that has been rectified as part of a previous casual pay adjustment.

Escalation process
If a casual pay claim is not resolved through the initial process, a former casual staff member may escalate the claim through a formal review.
